چکیده
STUDY OBJECTIVE: To evaluate the efficacy of hysteroscopic adhesiolysis and subsequent fertility in patients with adhesions stage 3 and 4. DESIGN: A retrospective cohort study (Canadian Task Force classification II-2). SETTING: A tertiary referral center for hysteroscopic surgery. PATIENTS: Seventy-one patients with intrauterine permanent adhesions. INTERVENTIONS: Hysteroscopic surgery with monopolar energy (n 31) or bipolar energy (n 40). Uterine cavity with at least one free ostial area was restored after one (n 31), two (n 20), three (n 15), or four or more (n 5) surgical procedures RESULTS: Sixty-four patients were followed. Evaluation of the uterine cavity after surgery has been performed by hysteroscopy for all the patients. All patients had resumption of menses, except for two patients with a history of uterine artery embolization. Pregnancy index rate after the procedure was 28 (43.8%) of 64, and the live birth rate was 21 (32.8%) of 64. In patients 35 years of age or younger, 20 of 30 (66.6%) conceived compared with 8 of 34 (23.5%) in patients older than 35 years (p .01). Three patients had either hysterectomy (n 2) or hypogastric arteries ligation for placenta accreta with uneventful postoperative course. CONCLUSIONS: Hysteroscopic adhesiolysis can be performed for severe adhesions stage 3 and 4 with safety and efficacy. Age is the main predictive factors of success: the pregnancies were at risk of abnormal placentation. © 2006 AAGL. All rights reserved.

The value of 3D ultrasound in the management of patients with suspected Asherman’s Syndrome
Objective: To assess the value of 3D ultrasound in the management of patients with suspected Asherman’s syndrome. Study Design: A case series of 54 infertile patients who presented to a tertiary care center, between 1998-2004, with suspected Asherman’s syndrome and underwent both HSG and 3D ultrasound prior to hysteroscopy. Sensitivity and the ability to attain fertility post-operatively were c...
متن کاملCXCL12 Promotes Stem Cell Recruitment and Uterine Repair after Injury in Asherman’s Syndrome
Asherman's syndrome is an acquired condition of uterine fibrosis and adhesions in response to injury that adversely affects fertility and pregnancy. We have previously demonstrated that bone marrow-derived mesenchymal stem cells (BMDSCs) contribute to uterine repair after injury and that stem cells supplementation improves fertility. Here, we demonstrate that CXCL12 is the chemokine that mediat...
متن کاملStimulation of decidua development by transplantation of endometrial stem cells
On all terms of pregnancy, insolvency of decidual reaction of endometrial cells is one of the reasons of miscarriages and fetal growth delay. The insufficient decidualization of endometrum leads to infertility in such pathologies, as Asherman’s syndrome and an endometrium atrophy. However, there are data on successful application of autologous bone marrow MSCs for Asherman’s syndrome treatment....
متن کاملP-248: Uterine Synechiae on Hysterosalpingography (HSG)
Background: This study showed the patterns and grading of Asherman’s syndrome on hysterosalpingography(HSG). Asherman’s syndrome (AS)which is termed as uterine synechiae, and intrauterine adhesions (IUA),result in trauma to the basal layer of the endometrium with subsequent scarring. This may be from previous pregnancy, dilation and curettage, surgery, or infection such as genital tuberculosis....
